How do I access the survey?

You will receive an email invitation to participate on Monday, Oct. 17 from surveys@safeandreliablecare.com. Your email will contain a unique link specific to you, so you must use the link sent to your email address to complete the survey and should not forward your link to anyone else.
You can also take the survey by scanning the QR code on flyers posted in your work area. You will need to use your employee ID number or physician ID number to enter the survey.

How long does it take to complete the survey?

The survey should take less than 15 minutes to complete. 

When does it need to be completed by?

The deadline to complete the survey is Monday, Nov. 7.

Do all employees need to complete the survey?

All employees - clinical and non-clinical - are encouraged to complete this survey. This survey is one way to give feedback about how we can become safer. You are all an important part of our organization. 

Are my responses confidential?

Yes, your responses will remain anonymous.

How is this different from the Best Places to Work surveys we are asked to complete?

Those surveys are only given to a small percentage of employees and the limited results do not give us a full picture of employees and internal culture. This survey is for all employees and addresses many areas including safety, communication, risk, resiliency, burnout, and engagement factors of your work environment.

In 2017, Safe & Reliable co-published its framework for high reliability with the Institute of Healthcare Improvement (IHI).
